# Form Assignments

Incident Tracker allows you to assign forms to specific categories, locations, or a combination of both. This ensures that users see only the relevant forms when submitting or editing a report.

***

## Accessing Form Assignments

To assign a form:

1. Navigate to **Admin > Form Assignments**.
2. Select the form you want to assign from the list.
3. Choose one or more categories and/or locations.
4. Click **Save** to apply the assignments.

***

## Assignment Logic

Forms will only display if the report meets the **assignment criteria**:

* If a form is assigned to a **category only**, it appears when that category is selected.
* If a form is assigned to a **location only**, it appears when that location is selected.
* If a form is assigned to **both a category and a location**, **both must match** in the report for the form to appear.

You can assign the same form to multiple category/location combinations for broader use.

## Best Practices

* Review your assignments periodically to ensure they match your reporting workflow.
* Use logical groupings (e.g., all Title IX categories) to reduce redundant form entries.
* Keep unused or outdated assignments clean to reduce clutter in the report interface.
